First, HIPAA!
In order for your insurance company, your provider, or anyone else handling your private health information (PHI) to discuss your claims with us, you will need to authorize them to do so. This document must be verified with identification, in our office.
What we need:
- Copy of the front and back of your insurance card(s).
- Copy of the bill you received from your health care provider.
- Copy of any EOB/Statement you received from your insurance company.
- Signed and verified PHI Release.
